This form should be completed to alert the CJCA to any incident that contravenes the CJCA Code of Conduct or detracts from the positive experience for Players, Volunteers, Supporters or Members of the Public. Should the incident be significant please involve the Junior Club Convenor for your Club or School so they are aware of the incident. Club contacts can be found here.
Incidents that result in injury that require admission to Hospital MUST BE REPORTED and Incidents that require treatment or that could classify as a “Near Miss” should be in the first instance be reported to the Junior Club or School Convenor so they can make an ‘Ecoportal” report which is viewed by governing bodies from CJCA to Canterbury and New Zealand Cricket.
